Class 1 - Preventive = Oral Exams, Prophylaxis, Flouride & X-Rays

Class 3 - Major = Inlays, Crowns, Dentures, Bridges, Repairs & Other Prosthetics

 

Chart Footnotes

1)  Plus Plans pay for Non-Network Dental provider services according to a Maximum Allowable Charge (MAC) fee schedule.  Non-network providers might charge more for certain services than the MAC fee schedule covers.  Therefore, members could be subject to balance billing when using Non-Network Dental Providers.

2)  PPO Plan Custom 1/114 pays for Non-Network Dental Provider services based upon Usual, Customary and Reasonable (UCR) fees.

3)  Services are covered at 100% except for the following, which have a co-pay fee:  office visit - $15; posterior composites - $25; crowns and bridges - $195 each; dentures - $195 per unit; root canals, extractions and deep cleanings - $35.

Underwriting Requirements

1)  Groups of 3-9 employees.

2)  Ineligible industries listed below:

    - Dentist offices and dental practices

    - Law firms & legal related SIC's

    - Membership Organizations

    - Bowling alleys

    - Car Washes

    - Taverns, nightclubs and non-classifiable establishments

3)  Rates assume no waivers for any reason, ALL employees must enroll.

4)  DHMO is not qualified as like coverage except when enrolling into a plus plan.

The Rates are Based on the Following Assumptions

1)  The employer pays 100% of the employee premium and 0% of the dependent premium.

2)  100% Participation by ALL eligible employees (required for all Super Small Groups Plans).
